“EC” DECLARATION OF CONFORMITY

The manufacturer, his authorised representative or, failing this, any person responsible for placing the product on the market established in the European Community (company name, full address; in the case of an authorised representative, also give the company name and address of the manufacturer)

declares that the model of new PPE-SL, described below (type, make, type, serial number, etc.)

is in conformity with the provisions of Section 4 of Chapter II of Title II of Book III of the EC Directive.)

(for all PPE-SL)

complies with the provisions of Section 4 of Chapter II of Title II of Book III of the regulatory part of the French Sports Code and, where applicable, with the national standard transposing the harmonised European standard (reference and date of the standard)

(in addition, for category 2 PPE-SL)

is identical to the PPE-SL which was the subject of EC type-examination certificate No.

issued by (name and address of the notified body which carried out the EC type-examination).

Le fabricant, son mandataire ou, à défaut, tout responsable de la mise sur le marché établi dans la Communauté européenne (raison sociale, adresse complète ; en cas de mandataire, indiquer également la raison sociale et l’adresse du fabricant)

déclare que le modèle d’EPI-SL neuf, décrit ci-après (genre, marque, type, numéro de série, etc.)

(pour tous les EPI-SL)

est conforme aux dispositions de la section 4 du chapitre II du titre II du livre III de la partie réglementaire du code du sport et, le cas échéant, à la norme nationale transposant la norme européenne harmonisée (référence et date de la norme)

(en plus, pour les EPI-SL de catégorie 2)

est identique à l’EPI-SL ayant fait l’objet de l’attestation d’examen “CE” de type n°

délivrée par (nom et adresse de l’organisme notifié ayant réalisé l’examen “CE” de type).
