INFORMATION AND DOCUMENTS TO BE PROVIDED BY LABORATORIES IN THE APPLICATION FOR RENEWAL OF APPROVAL

The application for renewal of approval must include the following information and documents:
1. Updated documents from the application file;
2. A report describing the activity (number of analyses performed) by category, during the period since the previous approval.
Three copies of the completed application for renewal of approval must be sent by post with acknowledgement of receipt to the public establishment mentioned in article L. 3512-15 of the Public Health Code.

INFORMATIONS ET PIÈCES À FOURNIR PAR LES LABORATOIRES DANS LE DOSSIER DE DEMANDE DE RENOUVELLEMENT D’AGRÉMENT

Le dossier de demande de renouvellement d’agrément comprend les informations et pièces suivantes :
1. Les pièces mises à jour du dossier de demande d’agrément ;
2. Un rapport décrivant l’activité (nombre d’analyses effectuées) par catégorie, pendant la période écoulée depuis le précédent agrément.
Le dossier de demande de renouvellement d’agrément complété doit être envoyé, par courrier postal avec accusé de réception, en trois exemplaires, à l’établissement public mentionné à l’article L. 3512-15 du code de la santé publique.
