The application for review shall contain, on pain of nullity:
1° For the applicants: an indication of their surnames, forenames and domicile. Where the interested party is deceased or declared absent, the applicant must indicate the surname and forenames as well as the date of death or declared absence and specify the capacity in which he or she relies on;
2° For defendants who are natural persons: the indication of their surnames, forenames and domicile.
For defendants who are legal entities: an indication of their form, name and registered office and, in the case of administrative or judicial authorities, an indication of their name and the place where they are established;
3° The constitution of the applicant’s lawyer at the Conseil d’Etat and the Cour de Cassation;
4° An indication of the civil decision handed down by a court of first instance or by the Cour de Cassation that is the subject of the application for review.
The statement specifies, where applicable, the heads of the decision to which the application for review is limited;
5° An indication of the decision of the European Court of Human Rights finding a breach of the European Convention for the Protection of Human Rights and Fundamental Freedoms or its additional protocols by the civil decision referred to in 4°.
It is signed by the avocat at the Conseil d’Etat and the Cour de cassation.
