The joint declaration provided for in articles 370-1-8 and 372 of the Civil Code shall be delivered or sent in triplicate by registered post to the registrar of the judicial court for the place where the child lives.
It shall be accompanied by the following documents:
1° A full copy of the child’s birth certificate and, where applicable, of the judgment granting simple adoption of the child;
2° For each of the parents, a full copy of their birth certificate and a copy of an official document issued by a public administration showing their surname, first name, date and place of birth, photograph and signature.
The registrar shall endorse and date each copy of the joint declaration. He shall notify, by registered letter, a copy to each of the parents in accordance with the conditions set out in articles 665 to 670-3 and shall keep a copy at the registry.
