Article 1180-1 of the French Code of civil procedure

The joint declaration provided for in articles 370-1-8 and 372 of the Civil Code shall be delivered or sent in triplicate by registered post to the registrar of the judicial court for the place where the child lives.

It shall be accompanied by the following documents:

1° A full copy of the child’s birth certificate and, where applicable, of the judgment granting simple adoption of the child;

2° For each of the parents, a full copy of their birth certificate and a copy of an official document issued by a public administration showing their surname, first name, date and place of birth, photograph and signature.

The registrar shall endorse and date each copy of the joint declaration. He shall notify, by registered letter, a copy to each of the parents in accordance with the conditions set out in articles 665 to 670-3 and shall keep a copy at the registry.

Article 1180-1

La déclaration conjointe prévue aux articles 370-1-8 et 372 du code civil est remise ou envoyée en trois exemplaires par lettre recommandée au directeur de greffe du tribunal judiciaire du lieu où demeure l’enfant.

Elle est accompagnée des pièces suivantes :

1° La copie intégrale de l’acte de naissance de l’enfant et, le cas échéant, du jugement prononçant l’adoption simple de l’enfant ;

2° Pour chacun des parents, la copie intégrale de son acte de naissance ainsi que la copie d’un document officiel délivré par une administration publique comportant son nom, son prénom, sa date et son lieu de naissance, sa photographie et sa signature.

Le directeur de greffe appose son visa et la date sur chacun des exemplaires de la déclaration conjointe. Il en notifie, par lettre recommandée, un exemplaire à chacun des parents dans les conditions prévues aux articles 665 à 670-3 et en conserve un exemplaire au greffe.
