Article 1180-7 of the French Code of civil procedure

The matter is referred to the judge by petition delivered or addressed to the registry of the judicial court. The application shall state, on pain of nullity, the surname, forenames and address of the applicant, his relationship to the minor, and the identity and address of the minor and his parents.

Where the application is based on the provisions of the second paragraph of article 387-3 of the Civil Code, it shall include on pain of inadmissibility, the particulars provided for in the previous paragraph and a precise statement of the facts likely to cause serious prejudice to the property interests of the minor or which manifestly and substantially jeopardise those interests as well as, where appropriate, the documents likely to justify these facts.

Le juge est saisi par requête remise ou adressée au greffe du tribunal judiciaire. La requête indique, à peine de nullité, les nom, prénoms et adresse du requérant, son lien avec le mineur, l’identité et l’adresse du mineur et de ses parents.

Lorsque la requête est fondée sur les dispositions du deuxième alinéa de l’article 387-3 du code civil, elle comporte à peine d’irrecevabilité, les mentions prévues à l’alinéa précédent et l’énoncé précis des faits de nature à porter gravement préjudice aux intérêts patrimoniaux du mineur ou qui compromettent manifestement et substantiellement ceux-ci ainsi que, le cas échéant, les pièces propres à justifier ces faits.
