Article 1218-1 of the French Code of civil procedure

The application for the protection of a person of full age provided for in article 1218 also mentions the persons belonging to the entourage of the adult to be protected listed in the first paragraph of article 430 and article 494-1 of the Civil Code, as well as the name of the person’s attending physician, if known to the applicant. As far as possible, the latter shall specify the details of the adult’s family, social, financial and property situation, as well as any other information, particularly relating to his or her autonomy.

La requête aux fins de protection d’un majeur prévue à l’article 1218 mentionne également les personnes appartenant à l’entourage du majeur à protéger énumérées au premier alinéa de l’article 430 et à l’article 494-1 du code civil ainsi que le nom de son médecin traitant, si son existence est connue du requérant. Celui-ci précise, dans la mesure du possible, les éléments concernant la situation familiale, sociale, financière et patrimoniale du majeur, ainsi que tout autre élément, relatif notamment à son autonomie.

Le greffier avise le procureur de la République de la procédure engagée, sauf lorsque ce dernier est le requérant.
