Article 1407 of the French Code of civil procedure

The application shall be made by petition delivered or addressed, as the case may be, to the registry by the creditor or by any agent. In addition to the particulars prescribed by Article 57, the application shall contain a precise indication of the amount of the sum claimed with a breakdown of the various elements of the claim, the basis of the claim and the list of supporting documents produced in support of the application. It is accompanied by these documents.

La demande est formée par requête remise ou adressée, selon le cas, au greffe par le créancier ou par tout mandataire. Outre les mentions prescrites par l’article 57, la requête contient l’indication précise du montant de la somme réclamée avec le décompte des différents éléments de la créance, le fondement de celle-ci ainsi que le bordereau des documents justificatifs produits à l’appui de la requête. Elle est accompagnée de ces documents.
