The initial application shall be made by writ of summons or by application delivered or addressed to the court registry. The application may be made jointly by the parties.
On pain of nullity, the initial application shall mention:
1° The name of the court before which the claim is brought;
> The subject of the claim
2° The subject matter of the claim;
3° a) For natural persons, the surname, forenames, profession, place of residence, nationality, date and place of birth of each of the applicants;
> b) For legal persons, the name, forenames, profession, place of residence, nationality, date and place of birth of each of the applicants
b) In the case of legal entities, their legal form, name, registered office and the body that legally represents them;
>The name of the legal entity and the date and place of birth of each applicant.
4° Where applicable, the particulars relating to the designation of the immovable property required for publication in the property register;
5° Where it must be preceded by an attempt at conciliation, mediation or participatory procedure, the steps taken with a view to an amicable resolution of the dispute or justification for dispensing with such an attempt.
