Any writ issued by a bailiff shall state, independently of the particulars prescribed elsewhere:
1. Its date;
2. a) If the claimant is a natural person: his surname, first names, occupation, domicile, nationality, date and place of birth;
b) If the claimant is a legal person: its form, name, registered office and the body that legally represents it.
3. The full name, address and signature of the judicial officer;
4. If the document is to be served, the name and domicile of the addressee, or, if it is a legal person, its name and registered office.
These particulars are prescribed on pain of nullity.
