Every European arrest warrant shall contain the following information:
the identity and nationality of the requested person;
the precise designation and full contact details of the judicial authority from which it emanates;
an indication of the existence of an enforceable judgment, an arrest warrant or any other judicial decision having the same force under the law of the issuing Member State and falling within the scope of the Articles 695-12 and 694-32;
the nature and legal classification of the offence, particularly with regard to article 694-32;
the date, place and circumstances in which the offence was committed and the degree of participation in it by the person sought;
the penalty imposed, if it is a final judgment, or the penalties laid down for the offence by the law of the issuing Member State and, as far as possible, the other consequences of the offence.
