Article 933 of the French Code of civil procedure

The statement shall include the particulars prescribed by 2° and 3° of Article 54 and by the third paragraph of Article 57. It shall designate the judgment appealed against, specify the heads of the judgment appealed against to which the appeal is limited, unless the appeal seeks to set aside the judgment or if the subject-matter of the dispute is indivisible, and mention, where applicable, the name and address of the appellant’s representative before the court. It is accompanied by a copy of the decision.

La déclaration comporte les mentions prescrites par les 2° et 3° de l’article 54 et par le troisième alinéa de l’article 57. Elle désigne le jugement dont il est fait appel, précise les chefs du jugement critiqués auquel l’appel est limité, sauf si l’appel tend à l’annulation du jugement ou si l’objet du litige est indivisible, et mentionne, le cas échéant, le nom et l’adresse du représentant de l’appelant devant la cour. Elle est accompagnée de la copie de la décision.
