Without prejudice to the list of long-term conditions set out inarticle D. 160-4 of the French Social Security Code, the list of chronic diseases, risk factors and situations of loss of autonomy giving entitlement to a prescription for adapted physical activity is set as follows:
1° Chronic diseases which have an impact on daily life, are likely to generate incapacity or even serious complications, and require long-term treatment;
2° Risk factors such as a condition, pathology or behaviour that increases the likelihood of developing or aggravating a disease or suffering from an injury, in particular overweight, obesity, high blood pressure, malnutrition, sedentary lifestyle, dyslipidemia and addictive behaviour;
3° The following situations of loss of autonomy due to disability or ageing:
a) The person is in receipt of the personalised autonomy allowance mentioned inarticle L. 232-1 of the Code de l’Action Sociale et des Familles (Social Action and Family Code );
b) The person is a beneficiary of the home help scheme referred to in article L. 231-1 of the same code;
c) The person is the beneficiary of a right granted by the Commission des Droits et de l’Autonomie des Personnes Handicapées (Commission for the Rights and Independence of Disabled People) referred to in article L. 146-9 of the same code;
d) The person is the holder of the “mobilité inclusion” card referred to in article L. 241-3 of the same code;
e) The person has undergone multidimensional screening for age-related functional decline revealing frailty.
