With the patient’s agreement, the masseur-physiotherapist may, unless otherwise indicated by the doctor, renew the initial medical prescription for adapted physical activity once the duration thereof or the number of sessions prescribed has expired, or adapt it in terms of type of activity, intensity, frequency and duration, on the specific form provided for in article D. 1172-2, under the following conditions:
1° The reports and assessments mentioned in article D. 1172-5 do not indicate any difficulty or risk in practising the adapted physical activity likely to harm the patient’s health;
2° The doctor involved in the treatment has not given any indication to the contrary either before or after the transmission of the report and assessments mentioned in article D. 1172-5 ;
The renewal of the medical prescription by the masseur-physiotherapist takes into account the proposals relating to the continuation of the activity contained in this report and the aforementioned assessments or adapts it to the needs of the patient.
The masseur-physiotherapist will record on the original of the specific prescription form, presented by the patient, the renewal or adaptation that he/she has carried out, indicating the following:
a) His full identification: surname, first name and identification number ;
b) The words “Renewal/adaptation (if applicable) of medical prescription for adapted physical activity”;
c) The date on which the masseur-physiotherapist carries out this renewal or adaptation, and his signature.
The original is given to the patient. The masseur-physiotherapist informs the prescribing doctor by any means which guarantees the confidentiality of the information transmitted.
