In order to provide the most appropriate equipment for the needs of the person who is ill or has an incapacity or disability, the service provider and the equipment distributor will ask the prescriber or the multidisciplinary team responsible for drawing up a personalised compensation plan for any additional information they deem necessary, particularly when they feel they have insufficient information on the development of the person’s pathology.

Where appropriate, and depending on the equipment supplied, the service provider and equipment distributor will inform the aforementioned prescriber or multidisciplinary team about the equipment supplied to the person with a view to monitoring it.

Afin de délivrer le matériel le plus adapté aux besoins de la personne malade ou présentant une incapacité ou un handicap, le prestataire de services et le distributeur de matériels demandent au prescripteur ou à l’équipe pluridisciplinaire chargée d’établir un plan personnalisé de compensation tout complément d’information qu’ils jugeraient nécessaires, notamment lorsqu’ils estiment être insuffisamment éclairés sur l’évolution de la pathologie de la personne.

Le prestataire de services et le distributeur de matériels informent, le cas échéant et selon le matériel délivré, le prescripteur ou l’équipe pluridisciplinaire précité sur le matériel délivré à la personne en vue de son suivi.
