I.- Information is provided by self-employed healthcare professionals and health centres:
1° By display in the places where patients are received ;
2° By prior quotation above a certain amount.
In the case of health establishments, information is provided by display in the places where patients are received and on public websites.
II.Where the procedure includes the supply of a custom-made device referred to in Article 1 of Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 concerning medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC, the standardised estimate includes the separate selling price for each product and each service offered, the corresponding responsibility rate and, where applicable, the amount of the excess charged and the amount reimbursed by the health insurance organisations.
In addition, the healthcare professional provides the patient with documents guaranteeing the traceability and safety of the materials used, where appropriate on the basis of information provided by a service provider or supplier.
III – The information put online by health establishments in application of the last paragraph of I may be included on the website of the Caisse nationale de l’assurance maladie and more generally by the public service mentioned in Article L. 1111-1.
