Article L3111-5 of the French Public Health Code

Any compulsory vaccination carried out under the conditions defined in this chapter must be the subject of a declaration by the doctor or midwife who carried it out, the terms and content of which shall be laid down by decree. This decree also lays down the procedures for transmitting to the National Public Health Agency the information required to evaluate the vaccination policy.

If the person vaccinated has a health record, the vaccination must be recorded in it.

Toute vaccination obligatoire pratiquée dans les conditions définies au présent chapitre doit faire l’objet, de la part du médecin ou de la sage-femme qui l’a effectuée, d’une déclaration dont les modalités et le contenu sont fixés par décret. Ce décret fixe également les modalités de transmission à l’ Agence nationale de santé publique des informations nécessaires à l’évaluation de la politique vaccinale.

Si la personne vaccinée dispose d’un carnet de santé, mention de la vaccination doit y être portée.
