Article L3212-5 of the French Public Health Code

I.-The director of the reception facility shall immediately forward to the State representative in the department or, in Paris, to the police prefect, and to the departmental commission for psychiatric care mentioned in article L. 3222-5 any decision to admit a person for psychiatric care in application of this chapter. It shall also immediately forward to this commission a copy of the medical certificate of admission, the entry form and each of the medical certificates mentioned in the second and third paragraphs of article L. 3211-2-2.

II – (Repealed)

III – In cases where the patient has been admitted in application of 1° of II of article L. 3212-1 or article L. 3212-3 and is being cared for in the form of full hospitalisation, the director of the host establishment informs the person who requested the care of any decision modifying the form of care.

Article L3212-5

I.-Le directeur de l’établissement d’accueil transmet sans délai au représentant de l’Etat dans le département ou, à Paris, au préfet de police, et à la commission départementale des soins psychiatriques mentionnée à l’article L. 3222-5 toute décision d’admission d’une personne en soins psychiatriques en application du présent chapitre. Il transmet également sans délai à cette commission une copie du certificat médical d’admission, du bulletin d’entrée et de chacun des certificats médicaux mentionnés aux deuxième et troisième alinéas de l’article L. 3211-2-2.

III.-Dans le cas où la personne malade a été admise en application du 1° du II de l’article L. 3212-1 ou de l’article L. 3212-3 et fait l’objet d’une prise en charge sous la forme d’une hospitalisation complète, le directeur de l’établissement d’accueil informe la personne ayant demandé les soins de toute décision modifiant la forme de la prise en charge.
