The health centres shall draw up a health plan covering, in particular, the accessibility and continuity of care and the coordination of health professionals within the centre and with external care providers.

The health centre’s operating rules shall be appended to the health plan.

The health project for a health centre managed by a health establishment is separate from the establishment’s project.

An order from the Minister for Health specifies the content of the health project and the conditions under which the centre’s health professionals are involved in drawing it up, as well as the content of the operating rules.

Article L6323-1-10

Les centres de santé élaborent un projet de santé, portant, en particulier, sur l’accessibilité et la continuité des soins ainsi que sur la coordination des professionnels de santé au sein du centre et avec des acteurs de soins extérieurs.

Le règlement de fonctionnement du centre de santé est annexé au projet de santé.

Le projet de santé du centre de santé géré par un établissement de santé est distinct du projet d’établissement.

Un arrêté du ministre chargé de la santé précise le contenu du projet de santé et les conditions dans lesquelles les professionnels de santé du centre sont associés à son élaboration ainsi que le contenu du règlement de fonctionnement.
