Article R.4122-4-22 of the French Public Health Code

In support of its application, the applicant must provide:

1° A declaration on honour to the effect that it does not fall into any of the cases mentioned in articles L. 2141-1 to L. 2141-11 of the French Public Procurement Code;

2° The information requested by the National Council for the purpose of verifying the applicant’s suitability to carry out the professional activity, its economic and financial standing and its technical and professional capacity, as well as documents relating to the powers of the persons authorised to bind them.

Where applicable, applicants must produce the documents specified in Articles D. 8222-5 or D. 8222-7 or D. 8254-2 to D. 8254-5 of the French Labour Code, as well as certificates issued by the relevant authorities and organisations proving that they have fulfilled their tax and social security obligations.

Le candidat produit à l’appui de sa candidature :


1° Une déclaration sur l’honneur pour justifier qu’il n’entre dans aucun des cas mentionnés aux articles L. 2141-1 à L. 2141-11 du code de la commande publique ;


2° Les renseignements demandés par le conseil national aux fins de vérification de l’aptitude à exercer l’activité professionnelle, de la capacité économique et financière et des capacités techniques et professionnelles du candidat ainsi que les documents relatifs aux pouvoirs des personnes habilitées à les engager.


Le candidat produit, le cas échéant, les pièces prévues aux articles D. 8222-5 ou D. 8222-7 ou D. 8254-2 à D. 8254-5 du code du travail ainsi que les attestations et certificats délivrés par les administrations et organismes compétents prouvant qu’il a satisfait à ses obligations fiscales et sociales.
