I.-The model referred to in article L. 1111-11, which may be used to draw up advance directives, includes:
1° The following information:
a) The identification details referred to in article R. 1111-17 relating to the author of the directives;
b) The identification details of the trusted support person referred to in article L. 1111-6 ;
c) Where applicable, details of the authorisations required in the case of legal protection measures with representation of the person, as referred to in the last paragraph of article L. 1111-6;
d) Where the person is physically unable to write his or her advance directives, the information relating to the two witnesses provided for in Article R. 1111-17;
2° The wishes of the person regarding medical decisions relating to the end of his or her life concerning the conditions for continuing, limiting, stopping or refusing treatment or medical procedures in the event that he or she is no longer able to express himself or herself. The model enables people to express their wishes in one of the following ways:
a) If the person is at the end of his or her life or knows that he or she is suffering from a serious illness, the person expresses his or her wishes concerning his or her possible future situation and the continuation, limitation, cessation or refusal of treatment and medical procedures, in particular those undertaken in the context of his or her illness ;
b) If he or she does not think that he or she is suffering from a serious condition, to express his or her wishes regarding his or her possible future situation and the continuation, limitation, cessation or refusal of treatment and medical procedures in the event that he or she is the victim of a serious accident or suffers from a serious condition;
3° A section enabling the person to express his or her wishes regarding the possibility of benefiting from deep and continuous sedation until death when, in the cases provided for by article L. 1110-5-2, the life-sustaining treatment is stopped;
4° A section relating to the revision or revocation of the advance directives.
II.-The model for advance directives, the content of which complies with the provisions mentioned in I, is specified by order of the Minister for Health.
III – Guides drawn up by the Haute Autorité de santé to help the public and healthcare and medico-social and social sector professionals to draw up advance directives based on the model mentioned in II can be consulted on the Haute Autorité de santé website.
