Article R1111-3 of the French Public Health Code

Where the request is imprecise or does not express a choice as to the means of communicating the information, the health professional or institution shall inform the requesting party of the various means of communication available under this section and shall indicate which will be used in the absence of a choice on his part.

If, at the end of the eight-day period or the two-month period provided for in article L. 1111-7, the requesting party has still not specified his wishes, the health professional or institution shall make the information available to him in the form previously indicated.

Lorsque la demande est imprécise ou qu’elle n’exprime pas de choix quant aux modalités de communication des informations, le professionnel de santé ou l’établissement informe le demandeur des différentes modalités de communication ouvertes par la présente section et lui indique celles qui seront utilisées à défaut de choix de sa part.

Si, au terme du délai de huit jours ou celui de deux mois prévu à l’article L. 1111-7, le demandeur n’a toujours pas précisé sa volonté, le professionnel de santé ou l’établissement mettent à sa disposition les informations sous la forme qu’ils lui avaient précédemment indiquée.
