Article R1112-3 of the French Public Health Code

The file includes the patient’s identification and, where applicable, that of the trusted support person defined in article L. 1111-6 and that of the person to be notified.

Each document in the record is dated and includes the patient’s identity with surname, first name, date of birth or identification number, as well as the identity of the healthcare professional who collected or produced the information. Medical prescriptions are dated, with the time indicated, and signed; the name of the signing doctor is indicated in legible characters.

Le dossier comporte l’identification du patient ainsi que, le cas échéant, celle de la personne de confiance définie à l’article L. 1111-6 et celle de la personne à prévenir.

Chaque pièce du dossier est datée et comporte l’identité du patient avec son nom, son prénom, sa date de naissance ou son numéro d’identification, ainsi que l’identité du professionnel de santé qui a recueilli ou produit les informations. Les prescriptions médicales sont datées avec indication de l’heure et signées ; le nom du médecin signataire est mentionné en caractères lisibles.
