Before discharge, all in-patients receive a questionnaire designed to collect their comments and observations. This completed questionnaire is returned to the administration in a sealed envelope and in anonymous form if the patient so wishes.
The Director periodically informs the Supervisory Board, the Institutional Medical Committee and the Institutional Technical Committee of the results of the use made of these documents.
These questionnaires are kept on file and may be consulted by the Directors General of the Regional Health Agencies.
