Article R1121-9 of the French Public Health Code

Subscription to the contracts mentioned in article R. 1121-4 is justified by the production of a certificate issued by the insurer, which constitutes a presumption of cover.

This certificate includes the following information

1° References to the applicable legislative and regulatory provisions ;

2° The name of the insurance company;

3° The number of the insurance contract;

4° The name and address of the policyholder;

5° The precise name of the research covered by the insurance as set out in the application for authorisation with its registration number, the planned start and end dates of the research as well as the number of people planned to be included and the name of the sponsor.

La souscription des contrats mentionnés à l’article R. 1121-4 est justifiée par la production d’une attestation délivrée par l’assureur qui vaut présomption de garantie.

Cette attestation comporte les mentions suivantes :

1° Les références aux dispositions législatives et réglementaires applicables ;

2° La raison sociale de l’entreprise d’assurance ;

3° Le numéro du contrat d’assurance ;

4° La dénomination et l’adresse du souscripteur du contrat ;

5° La dénomination précise de la recherche couverte par l’assurance telle que prévue dans la demande d’autorisation avec son numéro d’enregistrement, les dates prévisionnelles de début et de fin de la recherche ainsi que le nombre de personnes qu’il est prévu d’inclure et le nom du promoteur.
