Article R1221-49-2 of the French Public Health Code

I. – Any healthcare professional who observes or is aware of an adverse reaction occurring in a recipient of labile blood products shall report it without delay to the haemovigilance and transfusion safety correspondent of the healthcare establishment in which the product was administered. If he is unable to identify it, he will report it to any haemovigilance and transfusion safety correspondent in a blood establishment, who will forward this information to the relevant haemovigilance and transfusion safety correspondent.

II. – The haemovigilance and transfusion safety correspondent of the health establishment in which the product in question was administered carries out the appropriate investigations and examinations in the department concerned. He informs the correspondent of the referring blood establishment and, in consultation with him, draws up an adverse reaction report form for a recipient, a copy of which is placed in the recipient’s medical file.

I. – Tout professionnel de santé qui constate ou a connaissance d’un effet indésirable survenu chez un receveur de produits sanguins labiles le signale sans délai au correspondant d’hémovigilance et de sécurité transfusionnelle de l’établissement de santé dans lequel a été administré le produit. A défaut de pouvoir l’identifier, il le signale à tout correspondant d’hémovigilance et de sécurité transfusionnelle d’un établissement de transfusion sanguine, qui transmet cette information au correspondant d’hémovigilance et de sécurité transfusionnelle compétent.


II. – Le correspondant d’hémovigilance et de sécurité transfusionnelle de l’établissement de santé dans lequel a eu lieu l’administration du produit en cause procède aux investigations et examens appropriés dans le service concerné. Il informe le correspondant de l’établissement de transfusion sanguine référent et rédige, en concertation avec lui, une fiche de déclaration d’effet indésirable survenu chez un receveur dont copie est versée au dossier médical de ce dernier.
