Article R1413-72 of the French Public Health Code

Both parts of the form for reporting a serious adverse event associated with healthcare are sent by the Director General of the Regional Health Agency to the Haute Autorité de santé, in accordance with the procedures defined by order of the Minister for Health. This transmission does not include the full name of the reporter or the address of the place where the event occurred.

Les deux parties du formulaire de déclaration de l’événement indésirable grave associé à des soins sont transmises par le directeur général de l’agence régionale de santé à la Haute Autorité de santé, selon les modalités définies par arrêté du ministre chargé de la santé. Cette transmission ne comporte ni les nom et prénoms du déclarant ni l’adresse du lieu de survenue de l’événement.
