The centre referred to in the first paragraph of article R. 2141-3 shall compile a file containing information on the two members of the couple or the unmarried woman at the origin of conception which may be brought to the attention of a doctor, at his request, in the event of medical necessity pursuant to the fourth paragraph of article L. 2141-6.
This information relates in particular, in pseudonymised form, to :
1° The personal and family medical history of each member of the couple or of the unmarried woman and current clinical data deemed necessary by the practitioners ;
2° The results of the compulsory health screening tests provided for in article R. 2141-4 . The practitioner mentioned in article R. 2141-4 is responsible for keeping the file and ensuring the accuracy of the information recorded therein.
3° All information relating to the progress of pregnancies resulting from embryo reception, including any interruption, the date of birth and the state of health of newborns and children.
The pseudonymisation of the file and the allocation of the unique European code are carried out by the centre mentioned in article R. 2142-7.
The file must be archived under conditions that guarantee the confidentiality of the information it contains.
