The embryo register that must be kept by any health establishment, body, health cooperation group or laboratory authorised to store embryos must mention :
1° The identity of the couple or unmarried woman who is at the origin of the embryo and, where applicable, the unique European donation code or the anonymisation code of the gamete donor in the case of an embryo conceived with recourse to a third party donor ;
2° The number of embryos kept for each couple or each unmarried woman;
3° The place and dates of fertilisation and freezing;
4° Precise details of where the embryos are stored in the containers in the room allocated for this purpose;
5° Where applicable, the places where the embryos were previously stored;
6° Information relating to the future of each embryo, in particular the dates and results of the annual consultation of the members of the couple on the continuation of their parental project and the date of thawing of each embryo;
7° In the case of embryo reception, the unique European donation code.
