Article R2142-9 of the French Public Health Code

The medically assisted procreation centre keeps, under the joint responsibility of the authorisation holders and in compliance with confidentiality, the following information in the joint medical file mentioned in article R. 2142-8:

1° Indication of the implementation and choice of assisted reproduction technique ;

2° The date of gamete retrieval and the number of oocytes retrieved and prepared during each retrieval;

3° The date of the transfers and the number of embryos transferred;

4° Any available information relating to the fate of embryos, the progress of pregnancies and the state of health of newborns and children;

5° Any information relating to incidents and undesirable effects occurring during the implementation of medically assisted procreation.

Le centre d’assistance médicale à la procréation conserve, sous la responsabilité conjointe des titulaires des autorisations et dans le respect de la confidentialité, les informations suivantes dans le dossier médical commun mentionné à l’article R. 2142-8 :

1° L’indication de la mise en oeuvre et du choix de la technique d’assistance à la procréation ;

2° La date des ponctions de gamètes et le nombre d’ovocytes prélevés et préparés lors de chacune des ponctions ;

3° La date des transferts et le nombre d’embryons transférés ;

4° Toute information disponible relative au devenir des embryons, à l’évolution des grossesses et à l’état de santé des nouveau-nés et des enfants ;

5° Toute information relative aux incidents et effets indésirables survenus dans la mise en oeuvre de l’assistance médicale à la procréation.
