Article R2322-10 of the French Public Health Code

Establishments subject to the provisions of this chapter must keep, under the responsibility of the medical director :

1° A regularly updated register of visits, in which doctors and midwives record their medical observations and prescriptions and sign each visit;

2° Individual medical records for each patient and each newborn;

3° An operating protocol book in which surgical operations, voluntary termination of pregnancy, deliveries and resuscitation procedures are recorded.

In the event of transfer to another health establishment, a copy of the medical record must accompany the patient or the newborn.

Article R2322-10

Les établissements soumis aux dispositions du présent chapitre doivent tenir, sous la responsabilité du directeur médical :

1° Un cahier de visites régulièrement mis à jour dans lequel médecins et sages-femmes consignent leurs observations d’ordre médical et leurs prescriptions et apposent leur signature à chaque visite ;

2° Des dossiers médicaux individuels pour chaque entrante et chaque nouveau-né ;

3° Un cahier de protocole opératoire sur lequel sont inscrits les interventions chirurgicales, les interruptions volontaires de grossesse, les accouchements et les manoeuvres de réanimation.

En cas de transfert dans un autre établissement de santé, un double du dossier médical doit accompagner l’intéressée ou le nouveau-né.
