I.-For each child admitted, the micro-crèche’s technical advisor, the technical manager or the director of the establishment or service will ensure that the holders of parental authority or legal representatives provide the establishment or service with:
1° A medical certificate dated less than two months ago attesting to the absence of any contraindication to being cared for in a group setting. This certificate must be submitted at the time of admission and no later than fifteen days after admission;
2° A copy of the documents attesting to compliance with vaccination requirements, in accordance with the provisions of article R. 3111-8.
The establishment shall keep the documents mentioned in 1° and 2° until the end of the childcare contract.
II – At the time of admission, the technical advisor, the technical manager or the director, in conjunction with the “Health and Inclusive Reception” advisor mentioned in article R. 2324-39, will inform the child’s parental authority holders or legal representatives of the conditions under which the medical care and treatment mentioned in article R. 2111-1 may be administered to their child.
