I.- Establishments, services or organisations meeting the conditions set out in article R. 3115-64 and, in the absence of sufficient health resources, practitioners practising in French Guyana and meeting the conditions set out in article R. 3115-65 may be designated to carry out antiamaril vaccination .
II.-In support of their request for designation, the establishments, services, organisations or practitioners shall send the Director General of the Regional Health Agency a file containing :
1° The identity of the applicant;
2° Documents attesting to the qualifications and functions of the doctor in charge of the establishment, service or organisation, or of the practitioner practising in French Guyana and any information enabling their training and experience to be assessed;
3° A technical file enabling verification that the conditions mentioned in 4°, 5°, 6°, 8° and 9° of article R. 3115-64 or 3°, 4°, 5° and 6° of article R. 3115-65 have been met;
4° A sworn statement drawn up and signed by the applicant, the doctor in charge of the establishment, service or organisation, or the practitioner practising in French Guyana, stating that the other criteria mentioned in articles R. 3115-64 or R. 3115-65 have been or will be met.
III -The dossier accompanying the application is deemed to be complete if the Director General of the Regional Health Agency has issued an acknowledgement of receipt or has not informed the applicant, within two months of receipt, by registered letter with acknowledgement of receipt, of the list of missing or incomplete documents.
IV -The designation is made for a period of five years by the Director General of the regional health agency with territorial jurisdiction, in the light of the documents in the file accompanying the application if it is complete and, where applicable, after an on-site visit by an agent mentioned in Article L. 1421-1, within four months of receipt of the application.
V.-If there is no response from the Director General of the Regional Health Agency within the four-month period referred to in the previous paragraph, the application for designation is deemed to have been rejected.
