Article R3122-11 of the French Public Health Code

The request is made by written declaration delivered in duplicate against a receipt to the registry of the Court of Appeal or sent to the same registry by registered letter with acknowledgement of receipt.

The declaration must state the surname, first name and address of the applicant and the purpose of the application.

Where the statement of claim does not contain a statement of the grounds on which the claim is based, the claimant must file this statement at the court registry within one month of filing the statement of claim, failing which the claim will be inadmissible.

La demande est formée par déclaration écrite remise en double exemplaire contre récépissé au greffe de la cour d’appel ou adressée à ce même greffe par lettre recommandée avec demande d’avis de réception.

La déclaration indique les nom, prénom, adresse du demandeur et l’objet de la demande.

Lorsque la déclaration ne contient pas l’exposé des motifs invoqués, le demandeur dépose cet exposé au greffe dans le mois qui suit le dépôt de la déclaration, à peine d’irrecevabilité de la demande.
