Article R3332-6 of the French Public Health Code

Applications for approval must include :

– the name, legal status and address of the organisation ;

– extract no. 3 from the criminal record of the head of the organisation, dated less than three months before the date of application;

– a form in which the organisation certifies its economic independence from any establishment in the cafés, hotels, restaurants and discotheques sector, and from any company or organisation operating in the alcohol or tobacco sectors;

– the identity of each trainer, together with proof of their status;

– the training programme planned by the organisation;

– the number of people expected to attend each training session;

– the detailed training module ;

– the teaching aids ;

– materials given to participants;

– the price charged to each participant;

– the number, date and location of the sessions planned for one year in France.

The application for renewal of approval also includes the timetable for the sessions held and the number of participants.

Les demandes d’agrément comportent :

– le nom, le statut juridique et l’adresse de l’organisme ;

– l’extrait n° 3 du casier judiciaire du responsable de l’organisme, datant de moins de trois mois à la date de la demande ;

– un formulaire par lequel l’organisme atteste de son indépendance économique avec tout établissement relevant du secteur des cafés, hôtels, restaurants et discothèques ainsi qu’avec toute entreprise ou organisme exerçant dans les secteurs de l’alcool ou du tabac ;

– l’identité de chaque formateur ainsi que le titre justifiant de sa qualité ;

– le programme de formation prévu par l’organisme ;

– l’effectif prévu pour chaque session de formation ;

– le module détaillé de la formation ;

– les outils pédagogiques ;

– les supports remis aux participants ;

– le prix demandé à chaque participant ;

– le nombre, la date et le lieu des sessions prévues sur un an, sur le territoire national.

La demande de renouvellement d’agrément comporte en outre le calendrier des sessions réalisées et les effectifs accueillis.
