Article R3354-4 of the French Public Health Code

The officer or agent of the judicial police called upon to establish the traffic offence or accident shall immediately carry out a behavioural examination on the persons mentioned in article R. 3354-2, the result of which shall be recorded on a behavioural examination form known as form A, a copy of which shall be kept by the officer or agent.

In the event of death or serious injury that prevents the behavioural test from being carried out, this form simply indicates the circumstances of the offence or accident.

Article R3354-4

L’officier ou agent de la police judiciaire appelé à constater l’infraction ou l’accident de la circulation procède sans délai sur les personnes mentionnées à l’article R. 3354-2 à un examen de comportement, dont le résultat est consigné sur une fiche d’examen de comportement dite fiche A et dont il conserve copie.

En cas de mort ou en cas de blessures graves empêchant de procéder à l’examen de comportement, cette fiche se borne à indiquer les circonstances de l’infraction ou de l’accident.
