I.-An electronic traceability document is made available to all healthcare professionals, regardless of their status or mode of practice, on the website of the Agence nationale du développement professionnel continu (National Agency for Continuing Professional Development). This will enable them to keep a single personal record throughout their professional activity of the evidence of their commitment to continuing professional development as part of their three-year obligation.
The traceability document includes, with regard to the path defined in I of article R. 4021-4, the following elements:
1° Data relating to the identity of the professional ;
2° The various actions, classified in chronological order and according to the types defined in II of article R. 4021-4, which the healthcare professional has carried out;
3° Evidence that these actions have been carried out;
4° An annual and three-year summary of these actions;
5° Where applicable, the additional elements defined for his speciality or profession by the competent national professional council;
6° Where applicable, the document provided to the healthcare professional by his national professional council attesting to the compliance of the professional’s career path with its recommendations.
II – The traceability document provided for in I is a strictly personal document.
The healthcare professional is responsible for updating this document. He alone has the right to access, consult and write to it. However, in the case of healthcare professionals covered by the provisions of article L. 4138-2 of the French Defence Code, the Armed Forces Medical Corps has the same rights of consultation and writing as healthcare professionals.
At the end of the three-year period, the healthcare professional will send the authority responsible for monitoring his continuing professional development obligation a summary of the actions carried out. At any time, they may be asked to certify their commitment to the process, in accordance with the procedures laid down by the authority responsible for monitoring.
III – The data included in the traceability document is accessible in an aggregated and anonymised format, for the purposes of statistical analysis and impact studies on the system. This aggregated and anonymised data may be communicated to professional organisations, in particular national professional councils.
