Article R4127-79 of the French Public Health Code

Doctors must indicate on their prescription forms and other professional documents :

1° His surname, first names, professional postal and electronic address, telephone number and identification number in the shared directory of professionals working in the healthcare system;

2° His status with regard to health insurance organisations;

3° The speciality for which he is registered on the register or the qualification that has been awarded to him in accordance with the qualification regulations;

4° His membership of an approved association as provided for in article 371M of the General Tax Code.

It may also mention his titles, diplomas and functions when they have been recognised by the national council of the order, his honorary distinctions recognised by the French Republic, as well as any other indication taking into account the recommendations issued in this regard by the national council.

Le médecin mentionne sur ses feuilles d’ordonnances et sur ses autres documents professionnels :

1° Ses nom, prénoms, adresse professionnelle postale et électronique, numéro de téléphone et numéro d’identification au répertoire partagé des professionnels intervenant dans le système de santé ;

2° Sa situation vis-à-vis des organismes d’assurance-maladie ;

3° La spécialité au titre de laquelle est inscrit au tableau ou la qualification qui lui a été reconnue conformément au règlement de qualification ;

4° Son adhésion à une association agréée prévue à l’article 371M du code général des impôts.

Il peut également mentionner ses titres, diplômes et fonctions lorsqu’ils ont été reconnus par le conseil national de l’ordre, ses distinctions honorifiques reconnues par la République française, ainsi que toute autre indication en tenant compte des recommandations émises en la matière par le conseil national.
