Doctors must indicate on their prescription forms and other professional documents :
1° His surname, first names, professional postal and electronic address, telephone number and identification number in the shared directory of professionals working in the healthcare system;
2° His status with regard to health insurance organisations;
3° The speciality for which he is registered on the register or the qualification that has been awarded to him in accordance with the qualification regulations;
4° His membership of an approved association as provided for in article 371M of the General Tax Code.
It may also mention his titles, diplomas and functions when they have been recognised by the national council of the order, his honorary distinctions recognised by the French Republic, as well as any other indication taking into account the recommendations issued in this regard by the national council.
