Article R4241-8-1 of the French Public Health Code

Any person claiming the benefit of article L. 4241-9 must submit an application in duplicate to the Director General of the Regional Health Agency for the region in which he practises or last practised his profession:

1° A birth certificate or a copy of a valid national identity card or passport;

2° A certificate from the pharmacist or pharmacists who employed her or them, stating the date on which she began practising the profession of pharmacy assistant, and the length of time she has practised the said profession.

The conditions under which the pharmacy inspectorate will check the truthfulness of the documents provided by the interested parties are set by order of the Minister for Health.

La personne qui revendique le bénéfice de l’article L. 4241-9 adresse à cet effet une demande en double exemplaire au directeur général de l’agence régionale de santé de la région dans laquelle elle exerce ou a exercé en dernier lieu sa profession.A sa demande, elle joint :

1° Un extrait d’acte de naissance ou une copie de la carte nationale d’identité ou du passeport en cours de validité ;

2° Un certificat du ou des pharmaciens qui l’a ou l’ont employée, indiquant la date à laquelle elle a commencé d’exercer la profession de préparateur en pharmacie, ainsi que le temps pendant lequel elle a pratiqué ladite profession.

Les conditions du contrôle exercé par l’inspection de la pharmacie sur la sincérité des pièces fournies par les intéressés sont fixées par arrêté du ministre chargé de la santé.
