Any person claiming the benefit of article L. 4241-9 must submit an application in duplicate to the Director General of the Regional Health Agency for the region in which he practises or last practised his profession:
1° A birth certificate or a copy of a valid national identity card or passport;
2° A certificate from the pharmacist or pharmacists who employed her or them, stating the date on which she began practising the profession of pharmacy assistant, and the length of time she has practised the said profession.
The conditions under which the pharmacy inspectorate will check the truthfulness of the documents provided by the interested parties are set by order of the Minister for Health.
