Article R4321-122 of the French Public Health Code

The masseur-physiotherapist shall indicate on his prescription forms and other professional documents:

1° His surname, first names, professional postal and electronic address, telephone number and identification number in the shared directory of professionals working in the healthcare system ;

2° His diploma or degree entitling him to practise his profession;

3° His status with regard to health insurance organisations;

4° His membership of an approved association as provided for in article 371M of the General Tax Code;

5° Where applicable, his participation in a health network or care structure.

It may also mention his diplomas, titles, functions and specificities of practice when they are recognised by the Conseil national de l’ordre, honorary distinctions recognised by the French Republic, as well as any other indication taking into account the recommendations issued on the subject by the Conseil national de l’ordre.

Le masseur-kinésithérapeute mentionne sur ses feuilles d’ordonnance et sur ses autres documents professionnels :

1° Ses nom, prénoms, adresse professionnelle postale et électronique, numéro de téléphone et numéro d’identification au répertoire partagé des professionnels intervenant dans le système de santé ;

2° Son diplôme ou titre permettant l’exercice de sa profession ;

3° Sa situation vis-à-vis des organismes d’assurance maladie ;

4° Son adhésion à une association agréée prévue à l’article 371M du code général des impôts ;

5° Le cas échéant, sa participation à un réseau de santé ou à une structure de soins.

Il peut également mentionner ses diplômes, titres, fonctions et spécificités d’exercice lorsqu’ils sont reconnus par le conseil national de l’ordre, les distinctions honorifiques reconnues par la République française, ainsi que toute autre indication en tenant compte des recommandations émises en la matière par le conseil national de l’ordre.
