The declaration provided for in article L. 4331-6 shall be sent before the first service is provided to the Prefect of the region designated by order of the Minister for Health.
It shall include information relating to civil status, nationality, initial training, professional experience and lifelong training that has been validated by a competent body, language skills, the legality of the establishment in the Member State of origin or provenance, the absence of any ban, even temporary, on practising, professional qualifications, professional insurance and the place of performance of the first provision of services, as well as the list of supporting documents that accompany it.
The temporary and occasional nature of the provision of services is assessed on a case-by-case basis, in particular according to its duration, frequency, periodicity and continuity.
