I.- When renewing an orthoptic prescription, the orthoptist may, after carrying out a refraction examination and unless the doctor expressly states otherwise on the prescription, adapt the optical corrections of initial medical prescriptions for ocular contact lenses that are less than:
1° One year old, for patients aged under 16;
2° Three years old, for patients aged 16 and over.
II.-For the renewal of equipment, the orthoptist may, after carrying out a refraction examination and unless the doctor expressly states otherwise on the prescription, adapt the optical corrections of medical prescriptions for corrective lenses dating back less than:
1° One year, for patients aged under 16 years;
2° Five years, for patients aged between 16 and 42 years;
3° Three years, for patients aged over 42 years.
III.-The prescriber may limit the period during which the orthoptist may adapt the prescription by expressly mentioning this on the prescription, in particular in medical situations specified by order.
IV.- Orthoptists adapting initial medical prescriptions for corrective lenses or ocular contact lenses shall note on the prescription the correction adaptation that they carry out, legibly indicating their surname, first name, position, registration identifier carried out in accordance with Article L. 4342-2, date and sign this modification. He informs the prescriber by any means that guarantees the confidentiality of the information transmitted.
A copy of the modified prescription will be kept by the orthoptist until its validity expires and the original will be kept by the patient.
