Article R4354-10 of the French Public Health Code

The information sent, pursuant to article L. 4351-11 or article L. 4352-5, by the bodies issuing evidence of formal qualifications to the department or body responsible for registering the persons mentioned in article L. 4351-10 and article L. 4352-4 are:

1° The civil status details of the holder of the evidence of formal qualifications and any other identification details enabling the department or body responsible for registration to ascertain the identity of the applicant;

2° Where applicable, the name and address of the establishment or body which provided the training corresponding to the evidence of formal qualifications issued;

3° The title of the evidence of formal qualifications issued on completion of the training course, in accordance with the classification laid down by the regulations in force.

Les informations transmises, en application de l’article L. 4351-11 ou de l’article L. 4352-5, par les organismes délivrant les titres de formation au service ou à l’organisme chargé de l’enregistrement des personnes mentionnées à l’article L. 4351-10 et à l’article L. 4352-4 sont :


1° Les données d’état civil du titulaire du titre de formation et les autres données d’identification permettant au service ou à l’organisme chargé de l’enregistrement de s’assurer de l’identité du demandeur ;


2° Le cas échéant, le libellé et l’adresse de l’établissement ou de l’organisme ayant dispensé la formation correspondant au titre délivré ;


3° L’intitulé du titre de formation délivré à l’issue du cycle de formation, selon la classification prévue par les textes réglementaires en vigueur.
