Article R5121-188 of the French Public Health Code

In the establishments referred to in Article R. 5121-187, the person administering the blood-derived medicinal product to the patient completes the dispensing and administration slip with the following information:

1° The patient’s surname, forenames and date of birth ;

2° The date of administration;

3° The dose administered.

It mentions the dose administered and the date of administration on the prescription kept in the medical record and affixes a label detachable from the immediate packaging. She affixes the other detachable label from the immediate packaging to the form.

The completed form is immediately returned to the in-house pharmacy.

If it is not used, the medicinal product is returned to the in-house pharmacy with the form.

Article R5121-188

Dans les établissements mentionnés à l’article R. 5121-187, la personne qui administre au patient le médicament dérivé du sang complète le bordereau de délivrance et d’administration en y portant les informations suivantes :

1° Les nom, prénoms et date de naissance du patient ;

2° La date d’administration ;

3° La dose administrée.

Elle mentionne la dose administrée et la date de cette administration sur l’ordonnance conservée dans le dossier médical et elle y appose une étiquette détachable du conditionnement primaire. Elle appose sur le bordereau l’autre étiquette détachable du conditionnement primaire.

Le bordereau complété est immédiatement retourné à la pharmacie à usage intérieur.

S’il n’est pas utilisé, le médicament est retourné à la pharmacie à usage intérieur accompagné du bordereau.
