Article R5125-33-9 of the French Public Health Code

The pharmacist referred to in I of article R. 5125-33-8 registers the vaccine he administers, including when it is not included in lists I and II referred to in 4° of article L. 5132-1, under the conditions set out in articles R. 5132-9 and R. 5132-10, adding the information relating to the date of administration of the vaccine and its batch number. If no record is made, the pharmacist shall transcribe this information in accordance with the first paragraph of article R. 5132-9.

The pharmacist shall enter in the health record, the vaccination record or the shared medical record of the person vaccinated his full name, the name of the vaccine administered, the date of its administration and its batch number. If this information is not recorded, the healthcare provider will issue the vaccinated person with a vaccination certificate containing this information.

In the absence of a shared medical record and subject to the consent of the person vaccinated, the pharmacist will pass this information on to the person’s GP. This information is transmitted via a secure health messaging system that meets the conditions laid down in article L. 1470-5, where such a system exists.

Le pharmacien prévu au I de l’article R. 5125-33-8 enregistre le vaccin qu’il administre, y compris lorsque celui-ci n’est pas inscrit sur les listes I et II mentionnées au 4° de l’article L. 5132-1, dans les conditions prévues aux articles R. 5132-9 et R. 5132-10, en y ajoutant les mentions relatives à la date d’administration du vaccin et à son numéro de lot. A défaut d’enregistrement, le pharmacien transcrit ces informations conformément au premier alinéa de l’article R. 5132-9.

Le pharmacien inscrit dans le carnet de santé, le carnet de vaccination ou le dossier médical partagé de la personne vaccinée ses nom et prénom d’exercice, la dénomination du vaccin administré, la date de son administration et son numéro de lot. A défaut de cette inscription, il délivre à la personne vaccinée une attestation de vaccination qui comporte ces informations.

En l’absence de dossier médical partagé et sous réserve du consentement de la personne vaccinée, le pharmacien transmet ces informations au médecin traitant de cette personne. La transmission de cette information s’effectue par messagerie sécurisée de santé répondant aux conditions prévues à l’article L. 1470-5, lorsqu’elle existe.
