Article R5141-120 of the French Public Health Code

A veterinary surgeon who prescribes medicinal products containing substances having an oestrogenic, androgenic or gestagenic action for farm animals shall record the following information in chronological order in a register or by any appropriate recording system:

1° Number of the prescription prescribing the medicinal product in question ;

2° Nature of the treatment ;

3° Nature of the medicinal product ;

4° Date of treatment ;

5° Identity of the animals treated ;

6° Identity of the keeper of the animal(s) treated.

Le vétérinaire qui prescrit à des animaux d’exploitation des médicaments contenant des substances à action oestrogène, androgène ou gestagène consigne par ordre chronologique sur un registre, ou par tout système d’enregistrement approprié, les renseignements suivants :

1° Numéro de l’ordonnance prescrivant le médicament considéré ;

2° Nature du traitement ;

3° Nature du médicament ;

4° Date du traitement ;

5° Identité des animaux traités ;

6° Identité du détenteur du ou des animaux traités.
