Article R5141-126-1 of the French Public Health Code

The veterinarian registers the medicinal products referred to in the first paragraph of article R. 5141-126 using any recording system that allows immediate presentation at the request of the control authorities and does not allow any modification of the data after validation of the registration.

These records are kept for five years and include the following information:

-the name of the veterinary medicinal product and the batch number;

-the date of the prescription;

-the identification of the animals;

-the diagnosis;

-the dosage, the quantity prescribed and the duration of treatment;

-the route of administration and, where appropriate, the injection or implantation site;

-in the case of animals whose flesh and products are intended for human consumption, the withdrawal period, even if this is zero.

Article R5141-126-1

Le vétérinaire enregistre les médicaments visés au premier alinéa de l’article R. 5141-126 par tout système d’enregistrement permettant une présentation immédiate à la demande des autorités de contrôle et n’autorisant aucune modification des données après validation de leur enregistrement.

Ces enregistrements sont conservés pendant cinq ans et comprennent les renseignements suivants :

-le nom du médicament vétérinaire et le numéro de lot ;

-la date de la prescription ;

-l’identification des animaux ;

-le diagnostic ;

-la posologie, la quantité prescrite et la durée du traitement ;

-la voie d’administration et, le cas échéant, le point d’injection ou d’implantation ;

-dans le cas d’animaux dont la chair et les produits sont destinés à la consommation humaine, le temps d’attente, même s’il est égal à zéro.
