When the request comes from a beneficiary of the employment obligation who is self-employed, it is submitted using a form, the model for which is set by joint order of the ministers responsible for employment and disabled people.
This form, duly completed and signed, must be accompanied by the following documents:
1° Proof of status as a beneficiary of the employment obligation, as listed inarticle L. 5212-13 of the Labour Code;
2° Proof of registration as a self-employed person;
3° Proof of professional income for the previous year;
4° Proof of the costs incurred by the beneficiary in adapting his/her workstation and working environment to the best advantage;
5° Proof of the costs borne by the beneficiary in respect of ongoing expenses incurred as a result of their disability.
