For employees entitled to job subsidies leaving adapted companies mentioned in article L. 5213-13 of the French Labour Code , and users leaving establishments and services for assistance through work mentioned in article L. 344-2 of the Code de l’Action Sociale et des Familles , recruited by a mainstream workplace other than a sheltered workplace, the first decision to recognise the severity of the disability is taken on presentation of proof drawn up by the sheltered workplace, certifying that the person has been out of the workplace for less than one year on the date the application is submitted. This initial decision entitles the applicant to an increased rate of employment aid or, where applicable, to a reduction in the annual contribution provided for in Article L. 5212-9 under the conditions set out in Article R. 5213-51 . The model form containing the above-mentioned certificate is set by joint order of the ministers responsible for employment and disabled persons. This form, duly completed and signed by the employer or self-employed person applying for recognition of the seriousness of the disability, is accompanied by the supporting documents provided for in 1°, 2° and 3° of article R. 5213-42 or in 1° and 2° of article R. 5213-44 .
