The contract provided for in article R. 6146-17 is signed for a maximum of five years, renewable by amendment. The request for renewal is sent by the healthcare professional concerned to the director of the establishment no later than three months before the end of the contract.
The revision and renewal of the contract are approved by the Director General of the Regional Health Agency under the conditions set out in article R. 6146-17.
If the self-employed professional fails to honour his commitments, the director of the health care organisation may terminate the contract, either on his own initiative or at the request of the director of the regional health agency, after consulting the health care organisation’s medical committee. This measure is taken after formal notice has been given to the healthcare professional concerned.
The contract may be terminated immediately if the healthcare professional is subject to a criminal penalty, or a ban on practising for a period equal to or greater than three months.
